99214 CPT Code: Established Patient Office Visit
What 99214 covers
99214 is chosen when the encounter meets moderate MDM criteria. It requires at least two of the three key elements: a moderate number or complexity of problems addressed, a moderate amount or complexity of data reviewed and analyzed, or a moderate risk of complications, morbidity, or mortality. 99214 also applies when the total time spent reaches 30 to 39 minutes.
The total time encompasses all activities performed by the physician or qualified health professional on the encounter date. That includes reviewing records, ordering tests, counselling the patient, documenting, and coordinating care.
Importantly, 99214 is reserved for established patients seen by the same provider or group in the same specialty within the prior three years, excluding new patients.
When to Use 99214
Use 99214 for established patients whose conditions require moderate decision-making, such as managing two or more stable chronic illnesses with adjustment, one chronic illness with mild exacerbation, or a new undiagnosed problem with uncertain prognosis.
Choose 99214 over adjacent codes based on these boundaries:
- 99214 Vs. 99213: Select 99214 when problems or risk cross into moderate. Ex: two stable chronic conditions vs. one self-limited problem.
- 99214 Vs. 99215: Bill 99214 unless there is high risk, such as severe exacerbation threatening function, extensive data, or hospitalization decision.
- 99214 Vs. Time-based lower codes: Bill 99214 when total time hits 30 minutes, even if MDM feels lower.
Documentation that Keeps 99214 Paid
99214 is a high-volume code that represents a reimbursement step-up. It therefore draws audit scrutiny from payers. They review whether moderate MDM or sufficient time is supported.
Countercheck your notes meet at least one pathway:
- Problems addressed demonstrate moderate complexity. For example, describe an acute illness with systemic symptoms and explain how it affects management and risk.
- Independent interpret tests, such as reviewing and interpreting an ECG or ordering unique tests that influence decision-making.
- Prescription drug management decisions. State the specific drug name, dosage adjustment or initiation, and the clinical rationale to justify treatment.
- The total time and key activities. Ex: Total time spent on this encounter is 37 minutes, including 15 minutes face-to-face with the patient and 10 minutes reviewing prior records and labs.
Common Denial and Audit Triggers
- Insufficient MDM Documentation: Notes are flagged as incomplete if you fail to demonstrate at least 2 of the 3 MDM elements.
- Upcoding Concerns: Downcode to 99213 when the documented complexity falls short of MDM requirements.
- Time Ambiguity: Highlighting an approximate time range, instead of the specific one results in denials.
- Template Cloning: Heavy reliance on cloned notes, with minimal individualized patient content, raises red flags in automated audit systems.
Telehealth Billing
99214 is payable via telehealth for Medicare and most commercial plans. Use modifier 95 for real-time audio-video telehealth and modifier 93 for audio-only. You can use it with POS. POS 02 applies when the patient is not in their home. POS 10 applies to home telehealth. Confirm that your providers are properly credentialed with payers to ensure smooth reimbursement for 99214.
How 99214 Pairs with Other Codes
99214 pairs with appropriate add-ons, such as procedures, using modifier 25 when significant, separately identifiable evaluation and management work occurs. You generally cannot combine it with lower-level E/M codes on the same date by the same provider. However, if the total time exceeds 39 minutes, add prolonged services code 99417. Check for conflicts with preventive visits or other same-day services, and remember that the G2211 complexity add-on may apply in qualifying cases.
Frequently asked questions
Can I bill 99214 at every session?
No, you cannot bill CPT code 99214 for every session. It must be supported by either MDM complexity or total time spent on the encounter, per current AMA CPT and payer guidelines.
What is the difference between 99213 and 99214?
99213 requires low MDM or a total time of 20–29 minutes. 99214 requires moderate MDM or 30–39 minutes.
Does 99214 require prior authorization?
It depends. For in-person office visits, prior authorization is rarely required. For telehealth delivery, some commercial payers and Medicaid managed care plans require PA, particularly for audio-only services.
